Service Dog Application
Service dogs provide a wide
range of skills that help their partners live a more
independent life. They can accompany their
partners into all public areas. If you are just
thinking about getting a service dog you can still fill
out an application. The questions we ask can help
you decide if a service dog is right with you. |

Companion Dog Application
Companion Dogs have all the skills of
a service dog, however they are used only in the
recipient's home, not in public areas. It is our
recommendation that if you are too young to receive a
service dog you apply for a companion dog. They
can help prepare you for a service dog in the future.

Puppy Raiser Application
Puppy raisers are highly committed
volunteers that take raise a future service dog for it's
first year of life. Puppy raiser do not have to
live in the immediate area but can live anywhere in the
U.S.. If you love puppies and want to help someone
live a more independent life, this is the job for you! |

Volunteer
Application
There are many ways to volunteer with
our organization including doing office work, helping
train dogs, and assisting with fundraisers. If you
are interested fill out this application and we will
find a job that best suits your skills and interests.
